POTENTIAL PRICE INCREASE DISCLOSURE: I understand that Plaza Travel will make every effort to honor the price as originally quoted, however, under certain circumstances additional costs by a supplier or government may be imposed due to an increase in the cost for one or more of the travel components. Prior to full payment, prices including USA air transactions could increase for certain travel services, including the following: a seat, carriage of passenger baggage, applicable fuel surcharge or an increase in a government imposed tax or fee. Prices including USA air transactions will not increase after making full payment, except for charges resulting from government-imposed taxes or fees. Certain suppliers reserve the right to increase prices to cover increased costs, fuel surcharges, tariffs and taxes, and to reflect fluctuations in foreign exchange markets. I understand that Plaza Travel advisors will make every effort to notify me of any pending cost increase of which they are made aware. If I (or anyone in my party) decide to change any portion of my/our confirmed arrangements prior to departure, Plaza Travel will attempt to assist me/us at an administrative charge of $150.00 per change. I expressly acknowledge acceptance of these conditions applicable to my purchase and authorize Plaza Travel to charge my credit card for such additional amounts. This consent applies to all travel arranged by Plaza Travel for me that includes scheduled air transportation, and may be revoked at any time with respect to future travel for which I have not yet made payment. Five (5) years following the date of execution, this consent expires.

Additional Notifications, Based on Your Upcoming Bookings

CRUISE LINE PASSENGER BILL OF RIGHTS: The Members of the Cruise Lines International Association (CLIA) adopted the following set of passenger rights:
  1. The right to disembark a docked ship if essential provisions such as food, water, restroom facilities and access to medical care cannot adequately be provided onboard, subject only to the Master's concern for passenger safety and security and customs and immigration requirements of the port.
  2. The right to a full refund for a trip that is canceled due to mechanical failures, or a partial refund for voyages that are terminated early due to those failures.
  3. The right to have available on board ships operating beyond rivers or coastal waters full-time, professional emergency medical attention, as needed until shore side medical care becomes available.
  4. The right to timely information updates as to any adjustments in the itinerary of the ship in the event of a mechanical failure or emergency, as well as timely updates of the status of efforts to address mechanical failures.
  5. The right to a ship crew that is properly trained in emergency and evacuation procedures.
  6. The right to an emergency power source in the case of a main generator failure.
  7. The right to transportation to the ship's scheduled port of disembarkation or the passenger's home city in the event a cruise is terminated early due to mechanical failures.
  8. The right to lodging if disembarkation and an overnight stay in an unscheduled port are required when a cruise is terminated early due to mechanical failures.
  9. The right to have included on each cruise line's website a toll-free phone line that can be used for questions or information concerning any aspect of shipboard operations.
  10. The right to have this Cruise Industry Passenger Bill of Rights published on each line's website.

BAGS: Tips Related To Bags: http://airconsumer.dot.gov/publications/bagtips.htm  I know where my bags are checked, and realize my bags may be checked only to one of my intermediate stops rather than my final destination if:
  • I must clear Customs short of my final destination, or
  • I am taking a connecting flight involving two airlines which don't have an interline agreement (e.g., Southwest Airlines does not transfer bags to other carriers).
Hazardous Materials In Baggage: Beginning January 1, 2015, federal law requires Plaza Travel obtain each passenger's acknowledgement of the following notice before ticketing:

(1) US federal law forbids the carriage of hazardous materials aboard aircraft in my luggage or on my person. A violation can result in five years' imprisonment and penalties of $250,000 or more (49 U.S.C. 5124). Hazardous materials include explosives, compressed gases, flammable liquids and solids, oxidizers, poisons, corrosives and radio-active materials. Examples: Paints, lighter fluid, fireworks, tear gases, oxygen bottles, and radio-pharmaceuticals.

(2) There are special exceptions for small quantities (up to 70 ounces total) of medicinal and toilet articles carried in my luggage and certain smoking materials carried on my person. For further information contact the relevant airline representative(s) on your itinerary. Restrictions on hazardous materials are listed http://www.tsa.gov/traveler-information/prohibited-items.
INDIVIDUAL ENTRY AND EXIT REQUIREMENTS. Each foreign country holds different views of past criminal offenses, whether within or outside of their boundaries. If I or anyone in my party has a current or past offense, and we are unsure how the country we are traveling to (or through) views that offense, it is advised to contact that country directly for entry and exit requirements. (See: http://travel.state.gov/travel/cis_pa_tw/cis/cis_4965.html) For example, if traveling to or through Canada, Americans with a Driving While Intoxicated (DWI) record it is advised to always check whether current rules exclude admission, and potential waivers.
